Examination Retake Application
Used to apply for retaking an examination after failing to pass the examination on a previous attempt. All retake applications are now submitted online here.
Used to request replacement credentials. Common reasons include lost or damaged originals, or name changes (the Change of Records must be submitted to report a name change). Note: New credentials are issued without additional charge after each successful recertification cycle. This form should only be used if replacement credentials are desired before the next recertification cycle. Can be used at any time.
